چکیده

Cardiac output and arterial blood gases were studied before, during, and after intermittent positive pressure breathing, using 40% oxygen (IPPB i02). The subjects of this study were eight patients with lung disease, seven of them with chronic airflow obstruction and eight control subjects without respiratory symptoms. During IPPB/02, cardiac output was definitely lowered without a significant decrease in arterial Pco2 in the patients with airflow obstruction. Conversely, the control subjects during IPPB/02 had a large decrease in arterial Pco2 with a moderate reduction in cardiac output. Considering this response of the lung with airflow obstruction, the more frequent use of cardiac output and arterial gas measurements in patients receiving IPPB/02 treatments appears desirable. Individual cases are presented.

Respiratory and haemodynamic response to carbon dioxide breathing in patients with and without bronchial obstruction.

1. The effect of breathing a mixture of 5-6% carbon dioxide in air was examined in seven patients with obstructive bronchitis and in eight patients with lung disease but without bronchial obstruction. 2. Hyperventilation during CO2 breathing was less marked in patients with bronchial obstruction than in the others. Cardiac output did not change in patients with airways obstruction but rose in t...

Mask intermittent positive pressure ventilation in chronic hypercapnic respiratory failure due to chronic obstructive pulmonary disease.

Noninvasive ventilation in chronic obstructive pulmonary disease (COPD) has been shown to improve arterial blood gases but its long-term role has not been established. We retrospectively studied 26 consecutive patients with hypercapnic ventilatory failure due to COPD in whom oxygen therapy caused worsening hypercapnia (defined as a rise in the daytime arterial carbon dioxide tension (Pa,CO2) to...

Response of pulmonary circulation to oral pirbuterol in chronic airflow obstruction.

The effects of the oral beta agonist pirbuterol on pulmonary haemodynamics and gas exchange were studied in nine patients with severe irreversible airflow obstruction and moderate arterial hypoxaemia. After administration of 15 mg pirbuterol pulmonary vascular resistance fell by 19% but cardiac output rose by 24%, so that pulmonary arterial pressure showed no significant change. Systemic arteri...

Effects of fenoterol on oxygen transport in patients with chronic airflow obstruction.

A rise in cardiac output and a fall in arterial oxygen tension are well known side effects of bronchodilator drugs, particularly beta-adrenergic agonists. In recent years, fenoterol (Berotec), an effective beta-adrenergic agonist, has been used at increasing rates in asthmatic subjects, as well as in patients with chronic obstructive pulmonary disease (COPD). The effects of fenoterol on systemi...

Domiciliary nocturnal nasal intermittent positive pressure ventilation in COPD: mechanisms underlying changes in arterial blood gas tensions.

The improvement in arterial blood gas tensions following assisted ventilation in chronic obstructive pulmonary disease (COPD) has usually been attributed to the relief of incipient or established respiratory muscle fatigue. The contribution of changes in the load placed upon and the drive to the respiratory muscle pump have not been evaluated. We have investigated the contribution of changes in...
